Preparing for sixth grade is an exciting time for families and students! To ensure a smooth transition into the new school year, all students entering kindergarten and sixth grade must have required immunizations and submit the appropriate health documentation before the first day of school.

Kindergartners require:

  • Up-to-date and valid Kentucky Immunization Certificate with all required immunizations.


  • Kentucky Preventative Health Care Examination Form (School Physical).

  • Vision Exam – due by January 1st – must be performed by an optometrist or ophthalmologist.
    KRS 156.160 (1) (g) requires proof of a vision examination by an optometrist or ophthalmologist. This evidence

    shall be submitted to the school no later than January 1 of the first year that a three (3), four (4), five (5) or six (6)

    year old child is enrolled in public school, public preschool, or Head Start.

  • Dental Exam – due by January 1st:

    KRS 156.160(i), requires proof of a dental screening or examination by a dentist, dental hygienist, physician,

    registered nurse, advanced registered nurse practitioner, or physician assistant. This evidence shall be presented to the school no later than January 1 of the first year that a five (5) or six (6) year old is enrolled in public school.

The immunization certificate and physical exam are due on or before the first day of school. Please make appointments with your healthcare provider now!


Sixth graders require:

  • A physical examination performed within 1 year of entry into sixth grade. Please note that a KHSAA Sports Physical does NOT take the place of the required sixth grade physical exam. These are two different forms.

  • An up-to-date Kentucky Immunization Certificate showing all age-appropriate vaccines, which

    includes a tetanus vaccine (Tdap) and 1 dose of meningococcal vaccine (MenACWY).

The physical exam form and the updated Immunization certificate are due no later than the first day of 6th grade. I encourage you to make an appointment with your child’s healthcare provider as soon as possible to prevent delays.
